Dynamics of the perfusion foveolar parameters after silicone oil tamponade due to regmatogenic retinal detachment (macula-off)

Brief summary

When performing a pars plana vitrectomy for regmatogenous retinal detachment, a change in perfusion parameters, justified by the action of silicone oil and the nosology itself, is detected. If the integrity of the retinal block with the occurrence of retinal detachment occurs, a state of ischemia and subsequent retinal acidosis occurs, which leads to vasodilation and increased permeability in the area of retinal detachment in the preoperative period, primary retinal alteration in the central zone and the resulting state of ischemia leads to a gradual increase in the avascular zone of the retina, which is manifested by a significant increase in the area and perimeter of the FAZ on the 30th day in comparison with the data obtained in the early postoperative period (p = 0,041) and indicators of the control group (p = 0,037). The restoration of the foveal density capillary density index FAZ (foveal density - FD) is a compensatory mechanism in response to emerging ischemia, which is confirmed by an increase in this indicator on the 30th day in comparison with the indicators of the early postoperative period. The natural vitreous body is a buffer medium for dissolving and releasing ions. Thus, with an increase in the time of silicone tamponade, an increase in ischemia of the foveolar part of the retina occurs, while an increase in the area parameter of the avascular zone can be considered as a reaction to primary morphofunctional disturbances in the neuroarchitectonics of the macular zone.
