Constitutional and neurohumoral features in the premature cardiac contractions diagnostics in healthy individuals of later adolescence

Brief summary

To assess the diagnostic significance of some data of physical, laboratory and instrumental examinations for the detection of rhythm disturbances. Methods: Forty-four adolescents aged 15-19 years old (mean age 17.2 years) were examined. Using screening diagnostics, signs of connective tissue dysplasia were assessed. Performed ambulatory ECG monitoring and biochemical blood test. Results: In the examined patients, premature atrial contractions (PAC) and premature ventricular contractions (PVC) were detected in 98% and 43% of cases respectively. In 7.7% of cases, transient second-degree atrioventricular block type I was observed. In 73.1% of cases, sinus pauses with a duration of more than 1500 ms were detected. A weak negative correlation of the number of points from the screening diagnostics of connective tissue dysplasia (CTD) with the number of PACs (rs = -0.351, p = 0.011) and a weak direct relationship between PACs and PVCs (rs = 0.305, p = 0.028) was demonstrated. In the group with a larger number of dysplastic signs, the waist to hip ratio (WHR) and the level of total bilirubin were statistically significant lesser with respect to the group with a fewer CTD diagnostics points (p <0.05). In the group with a larger number of PACs (≥7 PACs per 24 hours) levels of urea (p=0.006), creatinine (p=0.04), total bilirubin (p=0.04), iron were lower (p = 0.0004). In persons who did not have a PVC during the 24h ECG monitoring, WHR was higher than in the group where at least one premature ventricular beat was registered (p = 0.003). Conclusion: Higher levels of serum iron and total bilirubin correspond to a lower frequency of premature atrial contractions. WHR take on larger values in individuals without ventricular ectopic beats.
