Stereotactic targeting of the deep brain structures using probabilistic mr-tractography methods

Brief summary

Purpose. Аssessment of the feasibility of using probabilistic tractography technology to visualize the cerebellar-thalamic tract under the conditions of using a standard diagnostic MRI scanner for stereotactic planning of functional neurosurgical interventions. Materials and methods. Three patients with Parkinson's disease underwent surgical treatment with deep stimulation (DBS). The study was based upon the results of this stereotactic targeting. To obtain data from diffusion-weighted tomography the technology of high angular resolution (HARDI) was used on a magnetic resonance tomograph Philips Achieva 3T. Tracts modeling was performed using a probabilistic algorithm of Constrained Spherical Deconvolution (CSD). The obtained outcomes were evaluated in overlaying the calculated connectivity maps on the digital stereotactic atlas of the thalamus and basal ganglia. Results. We were able to visualize the cerebello-thalamic tract using a standard MRI scanner. Stereotactic atlas and calculated connectivity maps data is comparable which indicates the possibility of using this technique in stereotactic planning of operations. Conclusion. Personalized approach to treatment of patients with movement disorders is obtained by using tractography methods in stereotactic planning of functional neurosurgical interventions. The implementation of this technique in stereotactic planning of surgical interventions will improve the effect of surgical treatment and postoperative management.
