Brief summary
The retrospective analysis of surgery at 130 patients at injury and degenerative diseases of cervical department of a backbone was carried out. At 58 victims (middle age of 29±5 years) the vertebral and spinal trauma, at 72 patients (middle age of 49±7 years) – degenerative and dystrophic diseases of a backbone took place. The diagnostic complex included an X-ray examination (survey and functional), MRI and CT of a backbone with a morphometry of various parameters of a vertebral segment, MRA and CT-angiography, and color duplex scanning of the magistral arteries of a neck. The algorithm of surgical treatment of degenerative diseases and trauma of cervical department of the backbone, based on massiveness of damage on horizontal (anterior, middle, posterior columns and their various combination) and vertical (number of the struck vertebral segments) levels taking into account a cross and longitudinal imbalance is offered. Strategy of surgical treatment of degenerative diseases the principle of «accruing radicalism» with five-step staging (conservative therapy, blockade, miniinvasive surgery, decompressive operations, decompressive-stabilizing operations and reconstructive interventions) was. Strategy of surgical treatment of a trauma liquidation instability using of dynamic and rigid fixing of a backbone, and their combination was.
